Guidance to filling in the form

If you get your nomination back before 15th December you may receive a £20.00 gift voucher to help with your Christmas Shopping (free prize draw).  The forms are much smaller this year - only 4 pages.  Much easier for you and your nominee.

This year there are some special sponsors - if you want to be considered for one of these awards just complete the standard nomination form but tick the right box or boxes** on the back of the each form.  You can be considered for more then one special award.
